Name: Actual name. When you join we'll be talking to you on ventrilo, and calling you by your IGN is lame.
IGN: In game name(s) so we know who you are.
Level: Classes / Base + Job.
Age: So we know.
Location: Where you're from + timezone so we know when to expect you online.
Hours online: So we know when and how long you'll be around.

Questions
What is your previous RO experience?
So we know what to expect. Can you do high end mvps? Do you know the tactics? How long have you been on RO?
Do you have work or school during WoE hours?
Just so we know. Exceptions can be made.
Can you attend WoE 90% of the time?
Required except for the above.
Can you dual client? Do you use BM?
Pretty straight forward.
Do you have a mic as well as ventrilo?
Again, straight forward questions.
Anything else you would like us to know?
